To produce a preliminary set of data to present to the Division of Public Health on cancer in Fayette County, citizens of the county are asked to complete this public survey. No names are needed.

The survey should have age, gender, race, contact info, subdivision or street name, city, type of cancer diagnosed, how long yu have been a resident in Fayette County and whether or not the cancer is primary or metastatic. Metastatic cancer is cancer that has spead or recurred.
